Procedures for the Neck and Chin (Jowl)
Facelift and neck lift surgery are often performed together. Deformities in the neck area are affected by many of the same factors that cause facial aging. Aging, skin type, gravity, and smoking are among the most significant factors. However, various complaints can also arise in the neck area. Sagging in the neck, folds extending from the chin downward, causing folds, fat accumulation in the chin area, and prominent horizontal lines on the neck are significant problems.
Necklift Surgery
Necklift surgery is performed under general anesthesia and in an operating room. The average surgery time is two hours. It involves reaching the SMAS layer through incisions in front of and behind the ears, suspending this layer, and thus creating tension in the neck. The incision scars are well concealed and heal almost completely. Often combined with liposuction, the surgery completely corrects sagging in the neck. The most significant complication in the early postoperative period is bleeding and the formation of hematomas. Therefore, the use of drains is recommended during neck lift surgery to rule out emergency intervention or as an early warning system. A one-week hospital stay is sufficient. What is Platysmal Sagging and Platysmoplasty?
Just under the skin of the neck lies a thin, fan-shaped muscle called the “platysma,” extending from the tip of the chin to the collarbones. The platysma muscle assists in neck movement and side-to-side rotation. Shortening (contracture) of the inner and then outer edges of the platysma muscle, which first face each other, manifests itself with aging as a pair of sagging lines (platysmal bands) that begin under the chin and extend toward the chest wall. This sagging progresses with age, taking on a fan-shaped shape.
In the treatment of platysmal bands, these muscles are accessed through a small incision under the chin. The contraction on the inner edges of the bilateral platysma muscles is incised in a suitable location to provide relaxation. The inner edges are then stitched together along the length of the neck, restoring an aesthetically pleasing neck arch (Platismoplasty). Platysmoplasty is often combined with submental liposuction or neck lift surgery. The incisions under the chin heal within weeks, leaving no scars. A 24-hour hospital stay is sufficient. Wearing a neck bandage for one week after surgery accelerates the healing process. Regular Botox injections in this area, while the platysmal bands are still forming, will delay the onset of the problem.
What is Submental Liposuction?
A frequently encountered problem is the accumulation of fat and folds in the chin, often known as the “submental area.” Fat accumulations under the chin can be easily and scarlessly removed with liposuction. The most important point to consider is whether the patient’s chin is in a normal position. In patients with a receding chin, the neck arch will not be visible, regardless of the amount of liposuction applied to the chin. In such cases, chin implants can be used to bring the chin forward. This procedure does not require hospitalization. A special bandage is used for one week after the procedure. The full results are achieved 4-6 weeks after surgery.
