What is Upper Eyelid Sagging?

The upper eyelids are composed of mucosa, muscle, cartilage, and skin, which help keep our eyes constantly moist and hydrated, and also serve a protective function. The detrimental effects of time manifest as sagging in the upper eyelids. In the early stages, eyelid sagging manifests as excess skin. This excess can create an aesthetically unsightly appearance but generally does not cause any functional loss. In more advanced stages, the levator muscle, which lifts the eyelid, also sags, causing problems such as reduced visual field, loss of attention, and uncontrolled sleepiness.

What Are the Consequences of Upper Eyelid Sagging?

Sagging upper eyelids is a condition that significantly affects facial aging. This is because the eyes and the area around them are the first facial features noticed during visual contact. Sagging skin on the eyelids can cause problems such as daydreaming and drowsiness when constantly looking at the same point (such as reading, watching TV, or driving). In the presence of muscle sagging, the eyelid droops, narrowing the field of vision.

Upper blepharoplasty is a surgical procedure that treats sagging skin and muscle in the upper eyelid. This procedure is usually performed under local anesthesia and takes approximately 45 minutes. Sometimes, only excess skin is removed, but if there is excess levator muscle, a portion of the muscle may be removed or folded back onto itself (resection or plication). It is extremely important to avoid aspirin for 1 week before the procedure and to avoid alcohol and herbal teas for 3 days beforehand. Intermittent ice application for the first 24 hours after surgery will prevent bruising and swelling. This procedure does not restrict the patient’s physical activities. Stitches are removed after 1 week.

What is Lower Eyelid Surgery?

The most significant problem seen in the lower eyelids is under-eye bags/herniated bags, accompanied by skin problems such as wrinkles and dark discoloration. This condition leads to a state of constant fatigue and negative energy.

How Do Under-Eye Bags Occur?

Our eyes are our most important sensory organ, connecting us to the outside world. The eyeball is protected within a bony socket called the orbit. Between the eyeball and this socket are pressure-absorbing fat pads, which protect the eye from the rigidity of the bone during movement and pressure. These fat pads are trapped within chambers called septums under certain pressure. Sometimes, a structural tear occurs in the septum, and the fat pads herniate forward under the pressure, creating under-eye bags. When pressure is applied to the eyeball, these fat pads are seen to herniate further forward.

How is Lower Eyelid Surgery Performed?

These fat pads are accessed through incisions made just below the eyelash line (subciliary). This line is an excellent location for concealing scars. The herniated area is excised and the torn septum is repaired. The skin is closed, and the excess skin causing under-eye wrinkles is carefully removed before the surgery is completed.

Because the lower eyelid is a very delicate area, the surgery is performed under general anesthesia. The surgery takes approximately one hour. It is recommended that the patient stay in the hospital for one night after the surgery, as uncontrolled bleeding in the early stages can lead to blindness. Due to the delicate and complex anatomical structure of the area, postoperative ectropion can occur, especially in heavy smokers. In such at-risk patients, it is possible to remove the herniated fat by entering the other herniated sac without removing any skin. The patient returns to a completely normal appearance within an average of two weeks after surgery.

Non-Surgical Eyelid Aesthetics

This recently frequently seen procedure involves tightening the eyelid tissue using a device. This can be likened to the shrinking of a plastic bag under heat. However, the effect is extremely limited and temporary. It has been known that patients often require further intervention within a short period of time.
