What is Midface Sagging?

The human face should be divided horizontally into three areas. The area from the hairline to the eyes is called the “upper face,” the area between the eyes and the lip line is called the “midface,” and the area from the lip line to the tip of the chin is called the “lower face.”

The visible effects of facial aging are first apparent in the midface. The downward displacement of the cheek tissue and loss of volume are the most significant changes. This change results in hollowing and sagging under the eyes. Furthermore, skin folds (nasolabial folds) extending from the wings of the nose to the corner of the mouth. Later, this sagging leads to a downward displacement of the corners of the mouth and sagging at the jawline.

Midface sagging can give the face a tired and aged appearance in the early stages. In this period, fillers applied to the cheekbones and nasolabial folds can temporarily correct the problem. Thread lifts can also provide more effective and long-term solutions.

Midface sagging can only be permanently corrected with a midface lift surgery.

How is a Midface Lift Surgery Performed?

What is the Recovery Process?

Midface lift surgery is performed under general anesthesia. The surgery takes approximately two hours.

Similar to lower eyelid surgery, a midface lift is performed with an incision extending just below the lower eyelash line (subciliary). This area provides excellent concealment of the incision scars. This allows us to perform lower eyelid surgery (blepharoplasty), correct herniated fat pads under the eyes, and address tear trough deformities all within the same surgery. In this surgery, the skin, subcutaneous tissue, and muscles that make up the cheek area are lifted from the bone and suspended from the bone surrounding the eyeball. The nasolabial folds or malar region are often supported with small amounts of fat injections.

Intense ice application in the early postoperative period can reduce swelling and bruising. Using a face mask for approximately one week from the outset has a significant positive effect on recovery. However, it is quite normal for swelling to occur around the eyes and face during the first week. It may take approximately 2-4 weeks for the patient to regain a natural and healthy appearance. A one-day hospital stay after surgery is usually sufficient. A midface lift is not a painful procedure. This procedure provides a significant facial rejuvenation.

The most significant early complication of a midface lift is bleeding, but it is very rare. Two possible complications, but not expected if performed correctly, are ectropion and permanent numbness of the cheek skin.
